Salivary parameters of individuals under systemic treatment with isotretinoin and its effect on the pH kinetics after exposure to acidic drinks

Abstract

Due to the multiplicity of protective functions exerted by saliva, under hyposalivation the clearing, diluting and buffering capacity of this fluid and its role in dental remineralization will be compromised. Considering that isotretinoin (Roaccutane®), prescribed for systemic use, causes xerostomia and hyposalivation, and that the effect of such a medicine on salivary parameters remains unexplored, this clinical trial, with observational and interventional components, aims to assess whether individuals undergoing treatment with isotretinoin have changes in flow, pH, buffer capacity and concentration of electrolytes in saliva. Considering that adolescents and young adults are the main groups of patients undergoing treatment with isotretinoin, and these are also the main consumers of acidic drinks and dental erosion is modulated by salivary aspects, this investigation also aims to monitor in vivo the kinetics of salivary pH of volunteers undergoing treatment with isotretinoin after the consumption of acidic drinks. After signing an Informed Consent Form, the stimulated and non-stimulated saliva of 40 volunteers (20 undergoing isotretinoin treatment and 20 not undergoing treatment, as control) will be collected to be evaluated for flow, pH, buffer capacity and the content of calcium, inorganic phosphate and fluoride. On three independent days, the volunteers will receive 15 mL of orange juice, cola-based soda (sugarless version) or a 10% sucrose solution (control). Immediately after the beverages had been sipped and in other 17 time points, a paladium miniature eletrode will be positioned in the central portion of the buccal aspect of the upper left central incisive and lower right second premolar to measure the pH on the dental surface. Data on different response variables (flow, pH, buffer capacity, concentration of electrolytes in saliva and kinetics of pH) will be submitted to descriptive statistical analysis, then, the inferential statistical methods, after being checked for compliance with the assumptions of parametric models (normality and homoscedasticity).
