Comparison of two methods of dental prophylaxis: evaluation of blood pressure and patient comfort: randomized controlled trial

Abstract

The participation of bacterial biofilms in the etiology of chronic inflammatory periodontal disease is extensively proven in the literature, so their removal and control are essential for the treatment of periodontal disease and the maintenance of oral health. Self-performed oral hygiene is an essential tool that patients have to control the biofilm, however, it is not an easy task, because of the oral anatomy, dental malposition, and it requires a certain level of manual skill to perform it properly. Thus, clinicians can help the patients keep the oral healthy by making more effective prophylactic methods for plaque control. Prophylaxis using bicarbonate jet has been used extensively in the clinic, replacing the traditional methods, such as the use of rubber cups and abrasive paste. Sodium bicarbonate is considered ideal for use in dental prophylaxis due to its abrasiveness, non-toxic properties. There is extensive literature evaluating its effect on enamel or restorations and soft tissues, however, little attention is paid to its effects regarding absorption by the mucosa and the possible changes in blood pressure. Thus, the present study will assess the change in blood pressure before and after two dental prophylaxis procedures: system bicarbonate jet and conventional prophylaxis and to evaluate the patient's opinion regarding the comfort of each one. 20 patients will be selected from the clinic of Periodontology FOSJC - UNESP, aged 18-30 years, who require prophylaxis to remove plaque, agree to participate, and sign the informed consent for this study. All patients will undergo to three different treatments: 1 rubber cup and abrasive paste; 2 water/bicarbonate jet; 3 placeboes: water jet without bicarbonate) at one-month intervals between them. Patients will be randomly assigned according to a computer-generated random list, which will determine the sequence of treatment that each patient will receive. A digital device WS Techline ® 501 before the procedure, immediately after the treatment, 15, and 30 minutes after treatment will measure blood pressure. The satisfaction and comfort of patients will be assessed through a visual analog scale (VAS) of 10cm at the end of each treatment. The plaque index and bleeding on probing be performed before treatment and the plaque index is repeated after treatment. A descriptive analysis of the data will be presented as mean and standard deviation. Data will be analyzed using analysis of variance with a significance level of 5%.(AU)
