Evaluation of hemorrhagic and thromboembolic disorders in dogs with chronic kidney disease

Abstract

Chronic kidney disease (CKD) affects mainly elderly dogs and has as its hallmark the irreversible loss of kidney function. This is responsible for several changes in metabolic processes. Chronic kidney disease in dogs induces severe metabolic disorders, often characterized by azotemia, hypoalbuminemia and nonregenerative anemia. Both azotemia and uremia predispose to hemostatic abnormalities, characterized by hipocoaguabilidade and/or hypercoagulability. Platelet dysfunction, natural anticoagulant deficiency and reduction of fibrinolysis are as predisposing factors associated with thromboembolism. This study aims to evaluate the possible hemorrhagic or thrombotic tendencies in dogs with CKD. Ten healthy dogs (control group) and ten dogs with CKD stages III or IV and proteinuria of glomerular origin (CKD group), aged between 2 and 12 years will be selected based on the results of complete blood cell count, determination of serum urea and creatinine, urinalysis and determination of urinary protein:creatinine ratio. The whole blood samples for thromboelastometry, platelet aggregation, prothrombin time (PT), activated partial thromboplastin time (APTT) and thrombin time (TT) will be collected in a single time for each group after the inclusion criteria were confirmed. 0,5mL of whole blood (EDTA K3 7.5 %) for complete cell blood count and 2,0mL of whole blood without anticoagulant will be collected for biochemical analysis. Two tubes containing 2,0mL of whole blood (3,2% sodium citrate) each will be collected for thromboelastometry, platelet aggregation test, PT, APTT and TT. The comparison between the groups will be performed by t test or Mann-Whitney test, according to the form of distribution, at 5 % significance level.
