Effect of combination of antibiotics and sinvastatin on endodontic microorganisms and its influence on the expression of odontoblastic markers

Abstract

In order to eliminate the bacteria and their toxins without damaging the adjacent periapical tissues, promote tissue regeneration and complete root formation in young permanent teeth, new alternatives to traditional intracanal medications have been evaluated. Some studies, mostly case reports, used the triple antibiotic paste containing metronidazole , ciprofloxacin and minocycline and found good clinical results associated with the antimicrobial properties of the paste. However, besides the bactericidal action of the medication, it would be interesting to include an agent to induce differentiation of residual undifferentiated cells, such as simvastatin, to stimulate the complete root formation. The objective of this study is to evaluate the antimicrobial activity of antibiotics alone or in combination on endodontic bacteria in planktonic and biofilm culture and determine the effect of the best combinations of these antibiotics associated with simvastatin on expression of genes related to the process of dentin mineralization. The antibiotics (metronidazole , ciprofloxacin , minocycline , doxycycline and fosfomycin) alone or in combination will be tested for antimicrobial activity against the following standard strains: Enterococcus faecalis, Streptococcus mutans , Actinomyces israelii and Candida albicans. Planktonic culture tests will be performed to determine the minimum inhibitory concentration (MIC) and minimal bactericidal concentration(MBC) of each antibiotic/combination. Biofilms of E. faecalis and C. albicans will be exposed to MBC of the best combinations of antibiotics. To assess the effect on dentin mineralization, cultures of pulp cells will be exposed to MBC of these combinations and analyzed the expression of genes DSPP and DMP - 1 by qPCR , alkaline phosphatase activity and production of mineralized nodules. The data will be analyzed in the statistical program, considering p<0.05. This study aims to contribute to the area of Endodontics and Pediatric Dentistry with the study of new biological alternatives that can promote the elimination of infection and tissue regeneration of immature permanent teeth.
