Translation, cultural adaptation and validation for use in Brazilian Portuguese of an international tool for supporting medical specialty choice

Abstract

Choosing a medical specialty is a complex process that affects not only medical students but also healthcare systems and organizations. Accordingly, many medical schools and healthcare organizations have specific services to advice medical students and physicians in regard to the choice of medical specialties as prospective careers. In the United Kingdom, many institutions use the "Specialty Choice Inventory", currently known as "SCI59," which consists of a 130-item self-administered, fully validated electronic questionnaire that produces recommendations on the medical specialties that best fit the personal profile of the candidate ("positive recommendations"), as well as on those specialties that less fit the candidate's characteristics("negative recommendations"). This project aims to translate and validate the "SCI59" for use in Portuguese by Brazilian medical students. The following main steps are planned: 1) translation into Portuguese and cultural adaptation of the original English version; 2) application of the translated and adapted Portuguese version to a number of medical specialists. The first step will consist of translation of the original questionnaire into Portuguese by two bilingual translators, followed by back translation into English of these Portuguese versions and an item-by-item selection of the best translation by the authors of the original inventory. The resulting questionnaire will be evaluated by a number of referees who may suggest changes to individual items in order to improve the questionnaire concerning clarity and readability. The second step will consist of the application of the resulting questionnaire to two subgroups of medical specialists: final year medical residents in well-defined specialties and established consultants working in the Brazilian National Health System and other private or cooperative healthcare organizations. These applicants will be asked to answer to a questionnaire on their demographic and personal characteristics. They will also be asked to indicate medical specialties that they would never choose. The requirements for validation of the Brazilian version of the "SCI59" will be the findings of concordance between the respondent's specialty and the "positive recommendations", as well as the concordance between the "negative recommendations" and the medical specialty they would never choose. It is expected that this work will provide Brazilian medical students, medical schools and other healthcare organizations with a new resource useful for advice on the choice of specialty, as well as with a new tool for research on the factors involved in medical specialty choice.
