Characterization of intracranial pressure by non-invasive method in post-stroke subjects: relationship with sedentarism and cardiovascular system

Abstract

The combination of a stroke and a sedentary lifestyle can lead to changes in the cardiovascular system and Cerebral Blood Flow (CBF), which may increase the risk of new cerebrovascular events. Concerning the regulatory components of CBF, there is the Intracranial Pressure (ICP). The development of new methods to monitor ICP brings clinical interest in intensive medicine practices, and in neurologic and cardiovascular rehabilitation programs. Non-invasive tools to track the ICP, bring fresh perspectives to help comprehend the cerebral hemodynamic patterns in chronic and incapacitating situations, such as post-stroke, beyond correlating to sedentary behaviors and changes in the cardiovascular system, like heart rate and blood pressure modifications. In this way, the primary goal of this project is to characterize the ICP and components of the cardiovascular system found in post-stroke individuals who exhibit sedentary behavior, comparing to healthy subjects, using these non-invasive methods. For this purpose, the project consists of a cross-sectional study with 33 subjects with at least six months post-stroke and 33 healthy subjects, who will be matched accordingly to their sex, age, and body mass index. The evaluations will occur on two separate days, with one week between each day. In the first day, only the post-stroke subjects will be assessed for sensory-motor impairment. Subsequently, for both groups, the 10-meter walk test will be done, and an accelerometer will be attached to the ankle of the individual and requested to be worn uninterruptedly for seven consecutive days. In the second day, the accelerometer will be removed to count the number of steps and the time spent in physical inactivity. Next, the level of physical activity, function of the autonomic cardiovascular system, represented by the heart rate variability and baroreflex sensitivity, and ICP presented by amplitude and latency through the active postural change maneuver, will be assessed. The data will be tested for normality and homogeneity. A two-way ANOVA or Kruskal Wallis test followed by the Mann-Whitney test will be used to compare differences between the hemispheres and groups, unpaired Student T-test or Wilcoxon test for the variables, and Pearson or Spearman coefficient to correlate the data. For all analyses, a significance level of 5% and confidence interval of 95% will be taken into consideration. (AU)
