CHARACTERIZATION OF THE INFLAMMATORY RESPONSE PROFILE IN THE PERIVASCULAR ADIPOSE TISSUE ASSOCIATED WITH CORONARY ARTERY DISEASE: AN AUTOPSY STUDY

Abstract

Introduction: Coronary artery disease (CAD) is the main cause of morbimortality in the world. In a previous study, we found a positive correlation between the number of macrophages in the perivascular adipose tissue (PAT) and the percentage of obstruction of the atherosclerotic plaque, and an association between B-lymphocytes and unstable lesions. However, it is necessary to elucidate the inflammatory response profile in the PAT associated with CAD. The analysis of the macrophage polarization was performed in few studies with surgical samples, which could initiate an inflammatory process. Moreover, the main lineages of the T and B-lymphocytes in human PAT have not been research yet. Aim: To investigate the association between the inflammation response profile in periplaque PAT adjacent to the atherosclerosis plaque in coronary arteries, and to compare this inflammatory response profile with PAT adjacent to the coronary artery without atherosclerosis (distal PAT), using autopsy material. Methods: This cross-sectional study will be conducted in 82 samples of participants previously collected in São Paulo Autopsy Service of the University of São Paulo. The coronary arteries were dissected, fixed in 4% paraformaldehyde, and microscopically evaluated using morphometric measures. The atherosclerotic plaques were classified in terms of instability according to the American Heart Association criteria. The samples of periplaque and distal PAT will be submitted to immunohistochemistry technique to identify macrophage polarization M1 (CD11c+) e M2 (CD206+), and double-marking to identify Th1 lymphocytes (CD4+CD183+), Tregs (CD4+FoxP3+), B1 (CD20+CD43+) and B2 (CD20+CD43-). The slides will be scanned and the inflammatory cells will be counted in 20 random fields systematically distributed. The inflammatory cells will be compared using linear regression models adjusted to possible confusing factors.
