Comparison of endothelial and cardiovascular function between prediabetic and normoglycemic individuals

Abstract

The vascular alterations in diabetics are already well elucidated, however, it is currently questioned whether the presence of pre-diabetes already exerts, in parts, changes in these individuals. The present study aims to compare endothelial function through arterial stiffness in healthy patients and in paired pre-diabetic patients by age and sex and correlate if the increase in arterial stiffness is associated with unfavorable parameters of cardiovascular function. METHODS: A prospective cross-sectional study was carried out with a sample of around 40 patients based on project innovation and controlled analysis of the methodology. Patients were divided into two Pre-Diabetic (PD, n = 20) and Normoglycemic (NG, n = 20). The medical students involved in the project will recruit patients from the primary health care units they serve based on the 2019 American Diabetes Association (ADA) pre-diabetic screening and diagnostic criteria: Fasting Plasma Glucose between 100 to 125 mg / dL, Oral glucose tolerance test between 140 to 199 mg / dL, Glycemic Hemoglobin (A1c) between 5.7 and 6.4%. The arterial stiffness will be evaluated by the non-invasive measurement of the carotid-femoral pulse wave velocity (PWV-cf), considered a gold standard by the American Heart Association (62), with the aid of the SphygmoCor XCEL equipment. Carotid ultrasound will also be performed to assess carotid medial-intimal thickness. Values will be expressed as mean and standard deviation or median and interquartile ranges. In variables with normal distribution, the categorical variables will be compared by the Chi-Square method and the continuous variables by the T-test. In variables with a non-normal distribution, a comparison will be made between the groups using Man-Whitney. The correlation between different variables in the same group will be performed by the Pearson test. Expected results: We expect to find an increase in arterial stiffness in pre-diabetic patients and that there is an association of this increase with greater mean carotid intima-media thickness, showing that these patients already have early markers of endothelial dysfunction.
