Applying RE-AIM framework to evaluate the implementation of PROERD in São Paulo

Abstract

The Educational Program of Drug Resistance and Violence (PROERD) is today the most widely applied school-based program in Brazilian schools and its dissemination will be exponentially expanded following the recent law that requires all public elementary and high schools in the state (São Paulo) introduce their students to the program. However, this expansion process is being carried out without any planning and/or evaluation of the program implementation process. PROERD in its current version is entitled "Caindo da Real", being translated in 2014 to Brazil from the US program "Keepin it REAL". It is based on the theory of social-emotional learning, which develops fundamental life skills, aiming to promote health behaviors and violence prevention. As preventive interventions developed in other cultures may have null or negative results when implemented in different contexts, the aim of this project is to evaluate the essential components of the process of implementation of PROERD/"Caindo da Real" (5th grade of Elementary School I and 7th grade of Elementary School II). A study of mixed methods (qualitative and quantitative) will be conducted using the RE-AIM model to describe the evaluation of implementation: Reach, Effectiveness, Adoption, Implementation and Maintenance. Phase 1 of the study will use data from two randomized controlled trials already conducted to assess Effectiveness through the effect of program mediators on drug use prevention and violence in the 5th and 7th years. Data were collected at two-time points through a self-completed questionnaire, one assessment before the application of the PROERD/"Caindo na real" program (February 2019) and another 9 months later (November 2019). Phase 2 of the study will be conducted in 2020 and will be conduct interviews, focus groups, classroom observations, analysis of program application material and analysis of program descriptive government data to assess the other axes of the program implementation process: Reach, Adoption, Implementation (Cultural Adaptation) and Maintenance. Descriptive analyses will be performed to assess the scope and adoption of the program and structural equation modeling to analyze the effect of mediators involved in the effectiveness of the program in preventing drug use and violent behavior. For the analysis of implementation and maintenance will be performed content analysis. The results will provide information on the need for modifications and improvements in the program implementation process that may support the expansion of the program, which became public policy in the State of São Paulo, by Law No. 17.171 of 2019.
