Abstract

Introduction: Non-communicable chronic diseases (NCDs), such as diabetes mellitus (DM) and systemic arterial hypertension (SAH), account for high morbidity and mortality rates in Brazil, especially among the elderly. Primary Health Care (PHC) is the main gateway to the Brazilian Unified Health System (SUS) for screening and monitoring these conditions. However, gaps in population coverage and quality of care compromise the effectiveness of public policies.Objective: This project aims to analyze how users with DM and SAH are screened and monitored in the PHC system of Campinas (SP), identifying bottlenecks and inequalities in the implementation of the care pathways established by the Health Care Network for People with Chronic Diseases.Methods: This is a quantitative study based on the analysis of secondary data extracted from the Primary Health Care Information System (SISAB), DATASUS, and the Brazilian Institute of Geography and Statistics (IBGE).Expected results: The study aims to identify the coverage and potential underreporting of DM and SAH, monitor the effectiveness of the care network, and provide recommendations to improve the system.
