Abstract

Mechanical ventilation is a method of treatment used in patients with acute respiratory distress. Although necessary, it brings a large number of complications, such as ventilator associated pneumonia and barotrauma. Thus, the early withdrawal of artificial ventilation is necessary. However, identifying the right moment to wean the patient from the ventilator is a challenge, because there is no consensus about the most efficient parameters to do so. In this context, automated weaning systems are useful, inasmuch as they permit constant surveillance of the patient's respiratory function and early identification of the right moment to withdraw the ventilatory support, thereby reducing the time of mechanical ventilation.Among the automated weaning modes, SmartCare deserves special attention because it is a system capable of diagnosing the patient's respiratory function through the analysis of the end tidal CO2 (etCO2), tidal volume and respiratory frequency. Based on the diagnostic, the system acts in order to mantain or adjust the pressure support, with the purpose of ensuring that the patient stays in a respiratory comfort zone.Even though there are studies proving the SmartCare efficiency, little is known about the system conduct in specific and particularly difficult situations of the ICUs. This work has the objective to study, in a lung model, the SmartCare performance in situations of normal and abnormal respiratory mechanics, anxiety/delirium, congestive heart failure with periodic respiration, elderly with periodic respiration and patient with pulmonary restriction.
