Abstract

In having transplanted renal, 60-100% of the patients develop infection for CMV, of which 20-60% present symptomatic infections. Several methods have been used to diagnostic the infection for CMV. These research have been used to illustrate the frequency with that it happens the reactivation of CMV powder-transplant and the clinical usefulness of monitoring the patients, allowed the direction of the clinical conduct to the patient. Recently, other factor has if shown important among them transplanted renal: the resistance incidence to the ganciclovir. Patients transplanted that were not exposed previously to CMV can be developed the disease for CMV under immunossupression, mainly the negative receivers that receive positive donor's organ besides the fact of being R-/D+ also a risk factor has been considered for the resistance development to the ganciclovir. Most of the isolated clinical resistant to the ganciclovir contains mutations in three ranches among the codon 400 and 707 of the area of UL97, and most of the mutations happens in the codons 460, 594 and 595. In Brazil, the mutation described more frequent it is L595S, and in other countries the more found is 460V. Several studies for laboratorial diagnosis to the ganciclovir resistance were developed, but in general they are difficult and the time for the diagnosis is long. The objective of this study is to verify the resistance incidence to the ganciclovir and to develop a quantitative PCR for detection and monitoramento of the infection for CMV in transplanted renal patient. A method that is of execution fast, easy, more sensitive and specific. This new method should be capable to evaluate the amount of resistant and wild type viruses in the same patient. (AU)
