Comparison and correlation between functional tests results and self reported questionnaire in functional verifications performed in knee osteoarthritis patients participanting in the efficacy of platelet rich plasma study

Abstract

Introduction: Osteoarthritis (OA) is the most common chronic condition of the joints and is characterized by cellular stress and extracellular matrix degradation. This process is followed by anatomical and/or physiological changes that are primarily, characterized by cartilage degradation, bone remodeling, osteophyte formation, inflammation and loss of function. To evaluate the efficiency of surgical or drug treatments as well as exercises, clinical studies usually consider pain and functions as analysis parameters. Despite several evaluation methods related to the treatment on knee OA patients, we can observe that in the vast majority of clinical trials that used Platelet Rich Plasma (PRP), only self-reported questionnaires were used. The use of several assessment methods on knee OA treatments, can provide an overall analysis; The main objective of this study was to compare and verify the interconnection between functional tests and WOMAC functional domain (DF) scores on knee OA patients, participating in a PRP treatment study, as well as the differences between the Groups, separated by age. Methods: For this study, the target population were men and women participating in a PRP efficacy study. The inclusion criteria were patients classified as moderate to severe knee OA (Grades II, III and IV) according to the Kellgren-Lawrence's grading scale, and aged between 18 and 80 years old. We included all patients who were selected for the study of PRP efficacy with a three-months follow-up. As methods of evaluation, were used: the DF of the WOMAC questionnaire and also functional tests (sit-and-lift test in 30 seconds, 40-meter walk test, Time up and go (TUG), Stair climb test and the 6-minutes walk test). Student t tests were used for dependent samples (parametric data) and Wilcoxon for paired samples (non-parametric data). To verify the difference between the groups, the ANOVA test and the Spearman rank correlation coefficients were used to analyze the interconnection between the WOMAC DF and the functional tests. The significance level was set as 5% (P <0.05) and all procedures were performed using SPSS. Results: The results showed that there is a difference between the pre and post functional performances in some functional tests as TUG and 30 seconds chair test for the total sample and also for the under 60 year-old age group, but for the over 60 year-old age group, it was not possible to verify the same difference in any of the functional tests before or after the intervention. The WOMAC DF showed a statistically significant difference in all groups. In the analysis of comparison between groups separated by age, there was a statistically significant difference in the TUG, stair climb test, 40 meters walk test, 6 minutes walk test in the pre intervention, and for all the functional tests in the post-intervention moment. However, in the WOMAC DF there was no difference between the groups at any moment. Analyses showed that there was a significant correlation between WOMAC FD and functional tests, except for those aged over 60 years old at the pre-intervention time. Conclusion: There is a correlation between the WOMAC DF and the functional tests used during this study. We believe that functional tests should be included as much as possible in the evaluation protocols because they show complementary results that may contribute to a robust analysis of the treatment benefits and also to the therapeutic interventions on knee OA patients (AU)
