Predictor factors of the cognitive outcome in subjects with mild cognitive impairment submitted to aerobic physical exercise

Abstract

The research project proposes to select a group (N = 80) of subjects with a diagnosis of Mild Cognitive Impairment (MCI), diagnosed according to the current literature criterion, and there may be amnestic deficit and / or not amnesic. The subjects will be invited to participate in an intervention of 24 weeks of aerobic physical activity attend, often twice a week and lasting approximately 50 minutes, performed at the Laboratory of Evaluation and Conditioning in Rheumatology (SEAL), located on the premises of the Hospital das Clinicas, Faculty of Medicine Universadade of São Paulo (FMUSP). Before training the subject will undergo a battery of cognitive testing (Mini Mental State Examination, modified ADAS-Cog, a brief cognitive assessment battery, symbol cancellation test, test tracks Part A and B, Test Extension in the order of digits forward and backward, Stroop, FAS verbal fluency and verb) and laboratory tests (dosage of BDFN - Brain Derived Neurotrophic Factor - in serum and CSF and serum amyloid protein, tau and phosphorylated tau in the CSF) and imaging (magnetic resonance volumetry of both hippocampus and proton emission tomography). In addition to the evaluations described above, they will be evaluated by cardiopulmonary exercise test before beginning physical activity. After 24 weeks, the subjects will be further evaluated by cardiopulmonary exercise testing, cognitive evaluation, proton emission tomography and measurement of serum BDNF.The answer provided by participants in the cognitive assessment (comparing before and after the intervention) will be correlated with the initial profile of the subjects in relation to biomarkers, and secondly we will measure a potential difference before and after the intervention with respect to proton emission computed tomography and BDNF serum.The project is based on the evidence in literature demonstrating the preventive character of aerobic physical activity in relation to cognitive decline. However, the neurobiology of this intervention is poorly understood and there is need for studies showing the profile of patients who can benefit and the factors that predict a beneficial response to aerobic exercise.
