Abstract

The aim of our study is to investigate the medial prefrontal cortex (MPFC), in modulation of chronic pain through possible activation of descendent pathway during the neuropathic pain induced by chronic constriction of the sciatic nerve (CCI). The role of MPFC and endogenous pain modulatory systems like periaqueductal gray matter (PAG), magnum raphe nucleus (MRN) and locus coeruleus (LC) will evaluate after stimulation of CCI. The neurostimulation of MPFC will be performer through of technique of deep brain stimulation (DBS) evaluating its effect on the neuropathic pain, and also record the activity of neuronal nuclei of endogenous pain inhibitory system. The nociceptive test will be used is mechanical allodynia, the von Frey at 7, 14, 21 and 28 days after stimulation of CCI. For the treatment of NP, it is very difficult to establish which drugs may be indicated and effective, and the drugs currently recommended for the treatment of NP still have an unpredictable efficacy, with only around 40-60 % of patients have partial relief pain. Neurostimulation is mainly applied in patients who do not respond to drug therapy. Interestingly , this device allows you to request up to four implant micro - electrodes, one electrode would be to perform the neurostimulation and the other three micro - electrodes would be to record neuronal activity. Thus, the project proposal is to implement a micro - electrode in the neocortex for stimulation of limbic cortex and other micro - electrode in the brainstem to the uptake of neuronal activity by neurostimulation of the neocortex. The device is innovative because it allows that the implanted microelectrodes sending or receiving information or registration stimulation of neuronal activity from the animal via a wireless by DBS, with a transducer with a small battery attached in the dorsal region of the animal, allowing the rodent to stay for free movement in the experimental apparatus which will assess the behavioral responses. In addition, I reaffirm that the contemplation of DBS for this project will be of great importance to address our hypotheses, increase quality of results and publications, establish collaborations with other research groups, including international, as with renowned Professor Sabatino Maione (Department of Experimental Medicine of Naples, Italy), which has eight patents and more than 250 articles in PubMed in the area of chronic pain and animal behavior. Maione will send a researcher from Italy to monitor the assembly and the use of DBS to FMRP-USP. This appliance is one of the essential ideas for a new methodology that intend to deploy in FMRP-USP. The DBS is also of interest to other professors, and then I will create Neuroelectrophysiological Multi User Center, where unit is in one of the rooms courtesy of Prof. Helio Rubens Machado. In our group of multi-user is comprised of nine Professors and coordinators of research groups. In fact , the professors have great interest in using this bold neuroelectrophysiological apparatus (see letters of the same ) and it will be available for me as multi-user via a trusted online scheduling in which all researchers involved in multi-user will have access. Thus, a reform in the space where the equipment will be allocated and more for DBS electrodes were requested so that other teachers can use it and keep the multiuser nature of this device that recommendation out of FAPESP . Also, I was reunited with the owner of the company that owns the patent and produces the DBS, Mr. Uwe Thomas, and he strengthened (see letter) that will send a technical team to support the implementation of multi-user equipment.
