A translational study for the identification, characterization and validation of severity biomarkers in arboviral infections

Abstract

Arboviruses, including those responsible for dengue fever (DENV), Zika (ZIKV) and chikungunya (CHIKV), are transmitted by Aedes mosquitoes and pose a large health risk. It is estimated that 390 mio dengue infections occur globally each year, of which 500,000 individuals require hospitalization due to the severity of symptoms. Severe forms of dengue fever (SDF) may present with a case-fatality rate of 20%. To date, there is no precise diagnostic tool to identify DENV-infected patients at risk for SDF. CHIKV presents with symptoms strongly resembling those of DENV, and severe forms of the acute infection are described in less than 1% of cases. However, CHIKV is more frequently associated with debilitating joint pain, affecting mainly wrists, ankles and elbows. In 4 to 63% of cases the joint pain becomes chronic and can last for months after the initial CHIKV infection. Prediction of persistent arthritis could change current treatment options. ZIKV has emerged in Brazil in 2015 and since then rapidly reached epidemic proportions. ZIKV has spread to over 40 countries and has been linked to Guillain-Barre syndrome as well as severe neurologic birth defects, leading the World Health Organization to declare ZIKV a public health emergency. A reliable test for early prediction of neurologic birth defects that could be assessed in biological samples of pregnant women could improve the implementation of interventions. The ARBOBIOS project aims to identify and validate prognostic biomarkers for DENV, ZIKV and CHIKV to enable early identification of patients at risk to develop the most unfavorable disease outcomes, SDF, severe neurological birth defects in newborns and chronic polyarthritis, respectively.
