Abstract

Type 2 diabetes mellitus (T2DM) is a relevant public health and obesity has been associated as the major risk factor. Roux-en-Y gastric bypass is an effective method to treat severe obesity, and its metabolic benefits lead to early and complete remission of T2DM, even before a considerable weight loss. Experimentally, the anatomical changes induced by RYGB can change genes expression in gastrointestinal tissue related to glucose control determinants to T2DM reversal. However, there aren't clinical studies that prove these findings in humans. Some gastrointestinal hormones already have been studied as possible mediators of glycemic control improvement after metabolic surgery, such as, ghrelin, peptide YY, enteroglucagon and incretins (GLP-1 and GIP). Due to endocrine effects of Roux-en-Y gastric bypass, it is reasonable to believe that other hormones and mechanisms are involved in glycemic homeostasis after surgery. The genomic, proteomic and metabolomics studies of gastrointestinal tissue in T2DM patients after Roux-en-Y gastric bypass can provide a better understanding of the T2DM pathophysiology enabling the study of new therapeutic targets for the treatment of this clinical condition.
