Abstract

Heart failure (HF) is a complex syndrome, considered as the leading cause of hospitalization in patients over the age of 60 years, and accounts for about 30 to 40% of mortality in patients with severe HF. It is also known that the neurohumoral excitation is a hallmark of HF and independent predictor of mortality in patients suffering from this syndrome. The sympathetic nervous system influence on the cardiovascular control depends on not only its tone, but also its modulation. Thus, both the frequency and the intensity of the sympathetic nerve discharge determine a pattern of oscillation that influences the efficiency of sympathetic effector response. This modulation of sympathetic nerve activity may be evaluated by the balance between the spectral components of low and high frequency of sympathetic nerve activity and represents the intrinsic behavior of the sympathetic nervous system functioning. It has been suggested that the lack of sympathetic modulation in a low frequency band predisposes patients with HF to higher mortality over a period of 12 months follow-up. However, the prognostic value of the oscillatory pattern of muscle sympathetic nerve activity (MSNA) in patients with HF is unknown. The proposal of this study is to test the hypothesis that patients with HF with reduced oscillatory pattern of MSNA present impaired survival and higher rates of hospitalization for a period of clinical follow-up of 12 months when compared with patients with higher oscillatory pattern of MSNA.
