Abstract Despite the association of aggressive periodontitis with known periodontal pathogens, particularly Aggregatibacter actinomycetemcomitans, and the involvement of host susceptibility with the periodontal, other differences in the composition of the oral microbiota may be involved with AP. Patients with aggressive periodontitis harbor low numbers of bacteria considered beneficial, particularly of the genera Streptococcus and Actinomyces, found in abundance in the oral biofilm of patients with normal periodontium. The initial adhesion of bacteria to dental surfaces is dependent of the composition of the enamel acquired pellicle, formed by proteins present in whole saliva. The present study aims to test the hypothesis that differences in the acquired pellicle composition between patients with aggressive periodontitis and normal periodontium are associated to differences in the composition of oral biofilm. Materials and Methods: The diversity of the supra and subgingival microbiota of patients with aggressive periodontitis will be analysed by 16srRNA sequencing and will be correlated with the profile of salivary and enamel acquired pellicle proteins. Five patients with localized aggressive periodontitis (PAL), less than 21 years old, carrying high levels of A. actinomycetemcomitans and low levels of other studied organisms will be selected. As control, 5 subjects with normal periodontium with no detectable levels of A.actinomycetemcomitans will be selected, matching to the aggressive periodontitis group in age, gender and race. Biofilm samples of supra and subgingival sites, parotid saliva, whole saliva and acquired pellicle will be collected. Analysis of supra and subgingival biofilm diversity of healthy and periodontitis sites will be performed after amplification of 16S rRNA followed by high throughput sequencing. The proteins profiles of parotid saliva, whole saliva and acquired pellicle will be determined by mass spectrometry and compared to the microbial composition of the supra and subgingival biofilm. Innovation: Differences in the composition of saliva among individuals can enable the discovery of proteins that promote colonization by beneficial bacteria instead of pathogens. These could be used in oral rinses in order to change the composition of dental biofilm, particularly in those families/populations with higher prevalence of aggressive periodontitis. In addition, beneficial bacteria associated with health can be identified to be used as probiotic agents against periodontal pathogens. Justification: Despite the low prevalence of aggressive periodontitis, this is a disease with family aggregation that leads to early tooth loss. So far, there are no effective preventive methods for patients from families with aggressive periodontitis. The mechanical treatment and antibiotic therapy are not completely effective, and should be performed throughout the life of the individual. Periodontitis is associated not only tooth loss, but is associated with various systemic conditions. This approach may in future be used in patients with chronic periodontitis, as stated in the grant proposal, simultaneously to this scholarship request. (AU) | |
