Quality analysis of the preparation and retreatment of different instruments associated or not to different protocols agitation irrigant

Abstract

The chemical-mechanical preparation of the root canal is an important steps of endodontic treatment and it becomes challenging in cases of teeth with curvature. Therefore, it is important that the instrument used at this stage is able to perform an adequate preparation while maintaining the original trajectory of the root canal. However, even before an adequate chemical-mechanical preparation and a correct filling, failures may occur and the endodontic retreatment can be necessary. However, after the removal filling procedure, material still remains in the canal. Therefore, irrigation protocols are used after the root canal filling removal for additional cleaning. Thus, the aim of this study will be to evaluate by means of computed microtomography (microCT), the quality of preparation and retreatment in curved canals, carried out with recently introduced instruments: Reciproc Blue, Wave One Gold, Prodesign R and BT-Race. Also, evaluate, using computed microtomography and scanning electron microscopy (SEM), the efficiency in removing residual filling material and cleanless of the dentinal wall promoted by different irrigating agitation protocols: ultrasound, Easy Clean alternating rotation and Easy Clean in continuous rotation. For this, 48 premolars showing curvature between 15 and 30 ° will be selected for microCT and scanned with predefined parameters. Then the teeth will be divided into 4 groups (n = 12) and instrumented canals with the following instruments: G1 - Reciproc Blue 40.06, G2 - Wave One Gold 35.07, G3 - BT-Race and G4 - Prodesign R 35.05. A new scan with the same initials parameters will be carried out and so, the transport of the canal, volume and areas not touched by the instrument will be evaluated. Once this is done, the canals will be obturated with the thermoplastic technique System B and Obtura II and AH Plus cement and then performed a new scan. After this procedure, canals will be submitted to endodontic retreatment with the same systems used in chemical-mechanical preparation. After the retreatment, a new scan will be performed to then carry out a new retreatment procedure, however, with an additional instrument of higher caliber of each system, that are: G1 - Reciproc Blue 50.05, G2 - Wave One Gold 45.05, G3 - BT-Race 50.04 e G4 - Prodesgin Logic 50.01. After that, it will be evaluated the filling material removal capacity, effective time of removal procedure and volume of filling material extruded through the apical foramen. After this, the canals will be divided into 4 groups (n = 12) and the following final irrigation protocols will be applied: conventional irrigation, Easy Clean alternating rotation, Easy Clean in continuous rotation and passive ultrasonic irrigation. After this procedure, there will be a scan of the teeth and will be evaluated for filling material removal capacity. Next, the teeth will be sectioned londitudinally and the hemisections will be evaluated by SEM, where the cleanless of the dentinal tubules will be observed. To determine the appropriate statistical test, the data obtained in each analysis will be submitted to D'Agostino normality test and Pearson, with the significance level of 5%. (AU)
