The effect of undiagnosed diabetes on association between self-reported diabetes and two cognitive domains: language and memory in adults over 50 years old: ELSI study

Abstract

Consistent evidence has indicated diabetes mellitus type 2 (DM2) as an important risk factor for cognitive changes in older adults. However, many studies have based on self-reported diabetes, not considering many people with DM2 who are not diagnosed. The purpose of this study is to examine the effect of undiagnosed DM2 on the association of self-reported diabetes with two cognitive domains, language (assessed by verbal fluency test), and memory (word list test) in people over 50 years old. For this, data will come from 9.412 participants of Brazilian Longitudinal Study of Ageing in 2015. Firstly, by self-report, the participants will be classified as diabetic (D) or non-diabetic (ND). Those who deny the diagnosis of diabetes but have glycated hemoglobin (HbA1c) greater than or equal to 6.5% will constitute the category of undiagnosed diabetics (UD), while those who deny having diabetes and have levels of HbA1c in normality (< 5.7%) or pre-diabetes (5.7% to 6.4%) will be classified as ND. Three multivariate logistic regression models will be used to analyses the effect of UD on the relationship between self-reported diabetes and cognitive changes. Model 1 will include the three categories of diabetes: D, ND, and UD. Model 2 will link UD in the same category as ND, and model 3 will include UD with D. These models will allow estimate the changes in these associations and all of them will be adjusted by control variables. The hypothesis is that non-separation from the group of undiagnosed diabetics may underestimate the association between self-reported diabetes and cognitive changes. (AU)
