Cardiovascular risk of children and adolescents with reumatic diseases: association with nutritional status and diseases severity.

Abstract

Context: children and adolescents with juvenile sistemic lupus eritemathosus (JSLE) and juvenile idiopatic arthritis (JIA) have high risk for cardiovascular disease (CVD), which is a major cause of morbimortality in these patients. Dislipidemic conditions, common in this group of individuals due to fisiopatological processes associated with the diseases, promote atherosclerosis in large arteries and, hence, increases the risk for CVD, and are intensified in conformity with disease severity. In JIA, the risk for CVD also depends on the disease's duration, subtipe and activity. Besides lipid metabolism modifications, glicid metabolism modifications and inflammatory biomarkers and image exams can straighten the diseases effects on patients organisms. Objectives: to describe the risk for CVD in these patients, associating with disease's severity and with body mass index Z score (BMIZ). Methods: it will be undertaken a retrospective study with children and adolescents of both sexes, with JSLE and JIA, in attendance at specialized ambulatory on Centro Universitário FMABC, with any clinical manifestations and degree of disease activity, that had given consent and assent in regards of the participation in the research, which will be accomplished by data collection from medical records, laboratorial exams to measure inflammatory and lipid and glicid metabolism biomarkers and Doppler ultrasonography to scout intima-media complex of the carotid (IMC). Expected results: as from the identification of risk for CVD in the research participants, it will be performed an awareness action with them and their legal guardians, with the aim of estimulating changes in the food habit and lifestyle of these patients. Conclusion: periodic screening of the risk is essential so that can be implemented therapeutic and changes in the food habit and lifestyle, with regular practice of physical activities, in order to reduce the risk for CVD and, therefore, morbimortality.
