Obsessive-Compulsive Disorder: pathogenesis and treatment

Abstract

Obsessive-Compulsive Disease (OCD) is a chronic disorder, affects 2 to 3% of the general population (Karno & Golding, 1991) and appears independently of sex, race, intelligence, civil-state, socio-economic status, religion or nationality. Despite the increasing number of research and publications, OCD remains an enigma. Its pathophysiology and heterogeneity of symptoms and clinical course remains insoluble. On the search for these answers, clinical, psychopathological, genetic, neurophysiological, immunological, pharmacological and neuroimaging studies have been demonstrating that OCD is a heterogeneous disorder which encompasses groups of patients with peculiar characteristics to each subgroup (Miguel et al., 1995). This diversity leads us to determine more homogeneous subtypes of patients and search for predictive factors of response to treatment. We believe that only this way it will be possible to elucidate important factors in the pathogenesis and then develop more specific therapeutic interventions, both pharmacological and non-pharmacological (Coffey et al., 1994).This project intents to study OCD, under many aspects: On module 1, we intent to investigate the heritability of OCD in a sib-pairs study. On module 2, an epidemiological genetic study, we intent to investigate the presence of a genetic relation between OCD and some diseases more commonly associated to it and Rheumatic Fever, a clinical disease that involves a streptococcal infection and lies been associated to OCD and to Tourette Syndrome. On module 3, a pharmacogenetic study, we are going to evaluate if there are genetic differences between responsive or non-responsive groups to clomipramine. On module 4, we are going to study the percentage of expression of D8/17 (a lymphocyte antigen that is found in almost 100% of patients with RF) in normal individuals, with OCD, OCD and RF, and RF alone (patients from module 1, 2 and 6).On module 5, we will investigate the cerebral blood flow of patients with OCD and RF (patients from module 2) to verify if there is any difference between flow patterns in comparison with patients with OCD without RF studied before. On module 6, we are going to investigate predictive factors of response to OCD treatment with clomipramine. This project gives also important emphasis on formation and training of new researchers as wellas on the development of young doctors on the beginning of their careers... (AU)
