A prospective study of radiation-induced trismus in patients with oral squamous cell carcinoma

Abstract

Patients undergoing treatment for oral cancer often present side effects from radiotherapy. Trismus is one of the most important complications of head and neck radiotherapy and can generate negative implications for affected patients, including food intake limitation, weight loss, impaired speech, limited oral hygiene and a decrease in the quality of life. The main goal of the current study is to diagnose and determine the incidence of radiation-induced trismus in oral squamous cell carcinoma patients undergoing radiation treatment. A prospective study regarding the mouth opening ability is going to be performed in such patients during oncologic treatment. In addition, a relationship among trismus, total radiation dose and different treatment modality is going to be established. Demographic and clinical information will be retrieved from patient`s medical records. Patients included in the sample are going to be divided in 4 groups according to treatment modality: tri-dimensional radiotherapy; modulated-intensity radiotherapy, concurrent chemoradiotherapy and control patients. Each group is going to be composed with 20 patients and mouth opening measurements will be performed once a week starting 7 days before to the commencement of treatment. Subsequently, 8 measurements of the mouth opening ability will be performed during the treatment (twice a week) and 1 year after the conclusion of radiotherapy. Mouth opening index (maximum vertical dimension and lateral mandibular movement) will be used according to the criteria of Dijkstra et al., 2006 and Kent et al., 2008 with the aid of TheraBite" ROM and OraStretch" ROM scales. Trismus gradation will be performed according to the new common toxicity criteria for adverse events version 4 (National Cancer Institute, Bethesda, Maryland, USA). Researchers enrolled in this study expect to contribute for the better understanding of the trismus issue in oncologic patients as well as to improve the treatment of this severe toxicity of head and neck radiotherapy.
