Xenogenous collagen matrix graft with or without enamel matrix proteins derivative in the treatment of Miller class I and II gingival recessions

Abstract

The purpose of this study is to evaluate histometrically in an animal model and clinically through a randomized controlled clinical trial, the healing pattern of single Miller Class I and II gingival recession defects treated by coronally advanced flap with subepithelial porcine collagen matrix graft and / or enamel matrix proteins. For the animal study, it will be included 8 minipigs BR1, males, weighing approximately 70Kg. After quarantine, the animals will undergo a surgical procedure to create the dehiscence type defects in the buccal of upper premolars (PII and PIII), bilaterally. After 1 month, following mucoperiosteal flap elevation, the defects will be randomly assigned to receive the treatments: 1 - CAF: coronally advanced flap; 2 - CAF associated with the porcine collagen matrix (CM); 3 - CAF associated with enamel matrix derivative (EMD); 4 - CAF associated with collagen matrix and enamel proteins derivative. After 12 weeks, the animals will be sacrificed and blocks will be removed for histometric assessment of the following parameters: total length of the defect; unfilled length of the defect; new cementum; new bone; periodontal regeneration; ankylosis, total epithelium length, connective tissue adaptation, new bone and new epithelium/connective tissue area. For the controlled clinical trial, 48 patients will be included with Class I and II Miller gingival recession in upper canines or premolars. Patients will be assigned into four groups, similar to the design of the animal study: 1 - CAF; 2 - CAF+CM; 3 - CAF+EMD and 4 - CAF+CM+EMD. Results will be analyzed by clinical by parameters: relative gingival recession height, gingival recession width, probing depth, relative clinical attachment level; keratinized tissue height and thickness, papilla height, root dentin hypersensitivity and aesthetic appearance. Statistical analysis comparing the treatments will be performed with ANOVA or Friedman test. The parameters with significant difference, it will be applied a post-hoc test with alpha of 5%. (AU)
