Impact of gastroplasty on salivary composition and cytomorphology of oral mucosa: follow-up three and six months

Abstract

The aim of this study is to evaluate the impact of gastroplasty in salivary physicochemical and microbiological composition, concentration of sulfurated compounds volatile related to halitosis and cytomorphology of the oral mucosa of 60 obese adults (18-40 years of age) undergoing gastroplasty (Capella technique) evaluated before and 3 and 6 months after surgery. Thirty normal weight (body mass index between 18.5 and 24.9 kg/m2), matched for gender and age, will be part of the control group. The physical examination includes measurements of height, weight, fat mass and skeletal muscle mass through a digital stadiometer and bioelectrical impedance analysis. DMFT and Community Periodontal Index will verify the oral health status. The self- perception of oral health- related quality of life will be verify using OHIP-14 questionnaire. Salivary amylase, total protein, calcium, uric acid and phosphorus concentrations will be determined in stimulated and unstimulated saliva by automated technique. Detection panel via Luminex will analyze salivary cytokines. Salivary DNA samples will be purified and subjected to quantitative PCR reactions (qPCR) in order to determine the proportion of Selenomonas noxia, Bifidobacteria, Streptococcus mutans and Prevotella intermedia in relation to the total bacterial load. The concentration of sulfurated compounds volatile [sulfhydrate (SH2), methyl mercaptan (CH3SH) and dimethyl sulfide (CH3)2S] related to halitosis will be verified using a gas chromatograph, Oral Chroma®. Micronucleus test will analyze oral mucosa cells. The description of the study variables and their correlation will be analyzed using descriptive statistics, normality test and correlation tests (Pearson or Spearman). Paired t-test/Wilcoxon will verify the changes resulting from bariatric surgery; unpaired t-test/Mann Whitney will be used for comparisons between groups. A multiple logistic regression model will verify the association of the variables under study with surgery (gastroplasty), controlling for confounders (± = 0.05).
