Influence of chitosan on the biomodification of eroded dentin: clinical and photographic longitudinal analysis of restorations

Abstract

The application of adhesive systems on eroded dentine may represent a challenge to restorative therapy, since the modifications that occur in the dentin surface, caused by the erosive process, compromise the introduction of adhesive in the organic dentin matrix, which decreases the quality and longevity of the restorations. The clinical application of biomodifying substances, such as chitosan, can improve the mechanical resistance of collagen, increasing its resistance to the degradation of collagen fibrils, used as support for the establishment of adhesive interfaces. Therefore, it is relevant to evaluate the influence of biomodification of eroded dentin by a biopolymer (chitosan), in order to improve its structural integrity and restorations. This study aims to evaluate the influence of biomodification with chitosan on erosion lesions by means of longitudinal and photographic analysis of the restorations performed. The sample will consist of 20 patients between 18 and 60 years of age who present non - carious erosion lesions of shallow or medium depth, located on the palatal or lingual surface of at least two homologous teeth (minimum - 40 teeth). The teeth of each patient will be randomly distributed into two groups according to the dentine surface treatment: 1) without biomodification (control) and 2) 2.5% chitosan. Conditioning will be performed with 35% phosphoric acid and then the cavity will be washed. For the group that will receive the biomodification with chitosan, chitosan will be applied at 2.5% for 60 seconds, and the surface will be dried with absorbent paper. The specimens will then be restored with adhesive (Adper Single Bond 2 - 3M) and composite resin (Z250 - 3M). For clinical and photographic analysis of the restorations, three calibrated and experienced professionals will use the modified United States Public Health Service (USPHS) criteria within 7 days (after polishing) and 6 months of restorations.
