Abstract

Antimicrobial resistance is an important public health issue. Multidrug resistant bacteria (MDR) are difficult to eradicate and their rapid dissemination lead to outbreaks and deaths in hospital environments. In Brazil, carbapenem-resistant Enterobacteriaceae (CRE), and specially those producing Klebsiela pneumoniae carbapenemase (KPC), are considered epidemic and mortality rates reach 48%. This scenario tends to worsen due to the lack of novel antibiotics. Studies that tackle the dissemination MDR bacteria are necessary. In this work we propose to identify endemic and sporadic bacterial clones in two hospitals located in Sao Paulo city that possess very different antibiotic and epidemiological surveillance programs. Genome sequencing will be used for the molecular characterization of CRE and to evaluate mechanisms of dissemination of resistance markers. We will also quantify the carriage load of CRE and KPC CRE using rectal swabs and associate results with infection and dissemination risk. Ultimately we aim to contribute with new strategies for the prevention and control of KPC CRE infections. (AU)
