Animal study protocol on immediate load on edentulous mandibles using replace select tapered implant with TiUnite surface

Abstract

Nowadays implantology determines great change in treatment plans proposed, being considered the first treatment option for edentulous patients, preventing the bone resorption and guaranteeing the remodeling and maintenance of the maxillary bones. The precocious load placement over the implants was recognized as critical factor for osseointegration, strengthening one of the reasons so why to respect the initial period of the bony healing. The premature load could carry to micro-movements that would result in encapsulation of the implant by fibrous tissue, fact that would prevent the adequate bony formation around of the implant. Authors suggested the occurrence of micro-movements varying between 10 and 20 µm and suggested, as well, which the tolerance limit would vary between 50 to 150 µm. The second reason refers to the necrotic bone around of the implant, originated by the surgical trauma. That tissue would not be able to absorb the masticatory loads and should be firstly substituted by neophormed bone. Other authors considered that the fast bony remodeling replacing necrotic bone by neophormed bone would pledge the resistance of the bone-implant interface. Nowadays countless histological and clinical studies allow reconsidering the conventional protocol of the Swedish school. The related results evaluating submerged and not submerged implants show similar effectiveness, as well as in the immediate activation of submitted implants to functional load. The selection of an ideal patient's is a very important factor, and should consider patient's general health, quality and bony quantity, oral hygiene, if the patient is a smoker or not, kind of occlusion and the specific region where the implant should be positioned. The present study will be accomplished with the objective to made a histological analysis of the periimplant tissues regarding the deposition of bony tissue and regarding the bone/implant contact, following the application of immediate or precocious (7 and 14 days) load in Replace Select Tapered implants with TiUniteTM surface (Nobel Biocare) installed in dogs jaws.
