Abstract

Hypertension (AH) is a multifactorial clinical condition characterized by sustained elevation of blood pressure levels e 140 and/or 90 mmHg. In the pediatric population, according to the European Society of Hypertension Guidelines for Children and Adolescents (2016), the increased prevalence of hypertension in this public has become a major public health issue, leading a significant amount of research. The rising prevalence of childhood obesity is an important predictor of obesity in adulthood and several other comorbidities. When it comes to hypertension, it depends on the interaction of genetic, biological and environmental factors, it is important to identify its prevalence, incidence and associated factors and this health outcome. The hypothesis is that the prevalence and incidence of blood pressure in South American children has worrying rates (>10%) and is associated with environmental factors (eg, walkability index) and lifestyle behaviors (eg example: physical activity and sedentary behavior). Objective: to verify the prevalence and incidence of high blood pressure in South American children and to associate with environmental indicators. Methodology: 12-month cohort follow-up with two time measurements. This project is part of a larger study called "New frontiers in pediatric nutritional and cardiovascular health: developing methods to evaluate double burden of malnutrition and optimal cardiovascular health in low-middle income countries - SAYCARE cohort study". Participants will be pre-school and primary students enrolled in public and private schools in their respective cities. The sample will be distributed proportionally to the size of the strata (gender and age) in four cities in South America: Buenos Aires (Argentina), Lima (Peru), Medellin (Colombia), Fortaleza, Sao Paulo and Teresina (Brazil). Measurement instruments will be used for this data collection: automatic oscillometric blood pressure monitors and different questionnaires. Systolic (SBP) and Diastolic (DBP) Blood Pressure and resting Heart Rate (HR) will be measured twice, with a two-minute interval between the two measurements, following the recommendations of the American Heart Association (AHA). If the values of the second measurement vary more than 5% from the first, a third measurement is required. Stata software version 15.0 (Stata Corporation, College Station, TX, USA) will be used for all statistical analyzes. Different statistical methods will be applied and for all hypothesis tests that establish the criterion of statistical significance at ± = 5%. (AU)
