Abstract Diets with high glycemic index, rich in liquid and refined carbohydrates, but low in fiber and whole grains are related to the development of insulin resistance and the inadequacy of micronutrients recommendations. Moreover, vitamins B6, C and E and the minerals zinc and magnesium play a role in the etiology of insulin resistance by acting on key metabolic pathways for normal insulin response. The aim of this study is to investigate the relationship between the carbohydrate quality index and micronutrient status and insulin resistance in adolescents, adults and elderly adults living in São Paulo. Data will be used from the Health Survey of São Paulo conducted in 2003, 2008 and 2015. It is a cross-sectional population-based study with a representative sample of adolescents, adults elderly adults in the municipality permanent homes. Demographic, socioeconomic, anthropometric and lifestyle data were collected on home visits and by telephone survey. Food consumption was investigated by two 24 hours dietary recalls. The quality of the carbohydrate will be evaluated using an index that takes into account four aspects of the nutrient: total fiber; glycemic index; ratio whole grains / (whole grains + refined grains or products); ratio solid carbohydrate / (solid carbohydrate + liquid carbohydrate). Single nucleotide polymorphisms related to insulin resistance will be evaluated and considered as adjustments in the analysis. The relationship between carbohydrate quality index, the biochemical status of vitamins B6, C and E and minerals zinc and magnesium and insulin resistance will be investigated by structural analysis equation.
